I believe its called "Photoshoppedtofuck"
Seriously would have no clue if it has a name or not. It looks like a consistent style so I'm sure there's something on the web about it, but It doesn't looks as simple as applying a preset effect

The genre of the images is Surreal/ Surrealism.
No idea about the digital processing.
photo surrealism
Yes some serious photoshopping and great imagination is used.
There are several books at my work on this.

the original was probably taken using strobes, then yeah, plenty of photoshop... few curve adjustments to get the leaky blue hue, not sure about those snowflakes.. .
if you want to shoot proper fashion-type shots, it's ALWAYS a team effort, you need a makeup artist/hair artist/usually an assistant to hold your reflectors and move your lights... you can go TF, but if you want to take good shots - it will cost you money.. :) at least until you get a name.. :)

I don't think there is really a name to describe this style other than photomanipulations or fine art portraits.
there are a few more photographers and graphic artists that do this.
If you wanna look at more work, check out Alexia Sinclair, Martin Grohs, Danielle Tunstall (more darker style, but amazing editing skills).

I used to use PTgui but now I find photoshop is good enough at stitching most of the time.
