Head gasket? Any kind of pinging under high load low rpm?

what model 4age? bigport, smallport, 20 valve etc. AFM or map? AW11's were a bigport 86kw with TVIS werent they? if you can get exacts on engine i might be able to help. spent heaps of time playing with 4a's. have you jumped TE1 and E1 to see what diagnostic code it is?

map sensor and model i'd assume bigport redtop. ok so in the engine bay you've got a diagnostic connector. fairly standard toyota pre obd setup. get a wire jumper and with the key off link the terminals TE1 and E1. turn key to on position and engine light will flash a series of codes. so for example 5 short flashes then long delay then 2 short flashes is error code of 52. possible causes for issue your having could be bad map sensor signal (check vacuum hoses), bad oxy sensor signal or more commonly bad coolant temp sensor. but hard to pin point without knowing the diag code

I would be pointing towards the AFM, TVIS is only on the big ports
not much that goes wrong on a GE.. do what daedausjc said and count the flashes
this will help you :)

yeah i first thought afm but its a map sensored engine. afm's were the yank delivered engine and i think the japs ran map in speed density. i had bulk issues in my 20 valve and aftermarket ecu missing over 9500 rpm all because of a tiny bump on the trigger wheel. bloody touchy but bullet proof engines
It has obviously been converted to MAP sensor as AW11's were the first gen GE/GZE's and came out with an AFM, they were also AFM from Japan. MAP's came out in the AE92's and it's common for converting the AFM and dizzy to MAP and CAS!
Have you checked all your vacuum lines as it sounds like you may have a leak? Does it idle fine?

do this as well
Put a hose clamp or multigrips on the fuel return line to reduce the fuel return so you can up the pressure in the rail..
sometimes when injectors get blocked they can stutter or bog down the car on take off
